I'm not really the person to ask, since it does nothing for me. I'm not grossed out by it, but... meh.

If I had to guess I'd say they're probably getting off on the "defilement" aspect of the kink, rather than self-inserting. Probably in a way that's similar to the monsterxgirl or insectxgirl genres of hentai.
