The story ends with Joel turning around gruffly to manfully face the sunset beardfully, and Ellie shoots him in the back of the head.

Once Ellie shoots Joel in the back of the head she then turns and walks back to the hospital to propose a radical new procedure which revolutionizes medicine forever, with the startling realization that you only need to take a tiny, tiny sample of cerebrospinal fluid to check for foreign bodies.
