Huh. I've never had that problem, I've only had one I didn't catch and that was it escaping the pokeball. Everything else I could catch as long as it was on the screen. But I got into it a day or two later than everyone else so maybe I just haven't played enough.

Probably! I live in a small area and everything's pretty close together.

No. I've been in a car on the highway (not as driver) and still been able to catch Pokemon. You do have to engage it while it's in your radius, but once you're in the "battle" with it, it doesn't matter how far you go away from the original encounter location.
