I'm in the USA, and I would consider all those incest EXCEPT second cousins.

Far from being squicks, these are bulletproof kinks for me in fic. But cousin incest isn't as erotic to me as the closer relationships because i lacks the stronger taboo element that lends the kink its eroticism.
