This is a complicated thing for me, because blood, whether or not they were raised together, and adopted/step played a big part of whether or not it makes me nope or not.

In real life:

If the couple has the potential to create children is a HUGE nope for me.

Blood related and raised together is also a huge nope.

Cousins/second cousins are case by case for me. Adopted and step siblings who were raised together are also case by case.

Adopted and step siblings that weren't raised together don't bother me at all.

Fiction:

Anything goes in fiction. There's some that I prefer over others, but real people aren't involved, so it doesn't bother me at all.

I'm from the US.
