In the finale of last season, Sam was shot by a representative of the U.K. branch of the Men of Letters, an organization that is dedicated to studying monsters and magic. (The US branch was taken out by one of S9's Big Bads.) Presumably (I've done my best to avoid spoilers for S12) Sam has been in fact captured and taken to England, and Dean is going to have to rescue him.
