Speaking english without a north american accent or with non-american vocabulary does not mean you speak it poorly!!!!!
You write english very well, I'd guess you also speak english very well. Please don't think people noticing your accent means you're not fluent, that's literally crazy talk.
I'm not trying to minimize your frustration with being understood, because I know that's real. Just remember that half my family lives in Australia and I can't understand them either :)
