I had a character do that once in a fic because I was new enough to the fandom to not realize nobody besides Jim ever called him that (and I'd seen a good amount of TOS at that point, it just...never occurred to me, especially since often the only other person who frequently interacts with him is Spock who isn't really going to use a nickname on McCoy). It wasn't because I thought the character was hot shit, though McCoy at least had no professional authority there, so it wasn't the inappropriate buddy-buddy thing you're complaining about.

Not OP, but I just wanted to say that I can see how people -might- think it's normal for everyone to call McCoy "Bones" -- after all, we the viewers do it all the time, so one can easily forget that was the nickname used exclusively by the Captain.

Yeah, in my own head I call him Bones, so it's like I have to be reminded it's only Kirk that calls him that.
