I remember this one because it was such a trainwreck.

The prompt was your basic h/c Thor/Loki prompt. Thor and Loki have a fight, Loki runs away, Thor goes after him and rescues him.

The fill started with Thor and Loki having a fight. Loki runs away to Midgard. Loki is rescued by a beautiful human girl. He falls in love with her. He realizes his relationship with his brother was an unhealthy one. He marries the human girl and has three kids Jordon, Finn, and Helen. The person who did the fill? One of the MODS. Who promptly closed it as filled and done.

When people started complaining that the fill was nothing like that the anon had asked for the mod got really pissy and started threatening to IP ban anyone who disagreed with her because no one was 'obligated' to stick the the prompt exactly.

Probably the most blandly disappointing was a Star Trek one, a friend asked for amputation fetish, Bones fucking between Scotty's fingers where he's missing the one. They got three replies worth of setup mutual pining with no sex and then the person filling it never finished it or got to the sex.
