Do weighted exercise that target your upper back and shoulders. Also yoga poses like eagle or, crud name just went out of my head, the one where you lay on your belly and lift your arms and legs. You need to stretch to release that knot. Or get a massage by someone who understands trigger points.

Locus pose. That was it. Also anything that does big, sweepy arm movements. You can also try using a doorframe as an anchor to stretch.
