re: #2, as another anon mentioned, sometimes it just happens because couple's have limited spare time and would like to spend it with each other, even if it's in the company of friends. sometimes it also happens because after a while it's just assumed that every invitation one half of a couple gets if for both of them - so one half of the couple not going can actually be seen as rude. I've had times when a friend has been annoyed with me for bring my boyfriend, and other times when they've been annoyed with me for not.

Considering the above, if you'd like to spend time with a friend without their SO, just ask clearly - 'hey, can we hang out just us two? i love hanging out with [insert SO's name here] but i've been really craving a girly/ one on one catch up lately!' And if the person refuses after you ask nicely, well then, maybe they are a weirdo.
