Honestly, hiring managers rarely call your "personal" references. I've absolutely put down my online friends before, and not one of them has ever been called, and they report the same. So you're probably okay there, as far as just putting down your online friends!

I'm really sorry about the vocational rehab issues, though. That's frustrating for sure. :(
