It is a little unnerving, but don't feel you can't speak up. My advice? If you're a virgin or small down there, make sure to ask them to use a small speculum (they do make them nowadays), and always make sure they use lube. Helps a lot with the discomfort (the only one who didn't was actually a female gyno).

The nurse practitioner at my doctor's office who's known me since I was four years old does my regular pap smears, and she's both good and reassuring.

I'm willing to put up with a little discomfort every three/five years vs not catching cervical cancer until it's too late.
