But doesn't it also have something to do with a pre-antibiotic era in which doing something that could cause a potentially fatal infection is more than just girlish vanity? There's the risk, particularly to a girl of delicate health like Rose (and by extension to a readership of potentially delicate girls) of becoming seriously ill.

No... that's absolutely not one of the reasons for the objection. "Death by ear piercing" was not a major concern of the era. Alcott just didn't like them.

Er, no. Rose's mother had pierced ears. Her similarly aged acquaintance ( a proper girl from a good family) has pierced ears. It's not considered a dangerous risk in that context, it's just Rose's uncle and cousins who are inflating the "danger". The book also makes it very clear that infection and illness is not their concern at all - it's that they regard it as a sign of female vanity and weakness.
