(Using the names from your example) Every doctor I've gone to I've started out calling Dr. Smith, and every one of them immediately says "Oh, call me Bob." At this point I feel a bit silly not using their first name right away, because I know they're just going to tell me to.

Same. The one doctor that I see the most is this big, jovial guy (think Santa Claus only without a beard) and he's very insistent that you call him by his first name.
