Are the patients older than the doctor? Some older people get super awkward about calling younger people by titles or Mr./Ms. It's like there's some kind of DOES NOT COMPUTE error in the social hierarchy calculation.

Now that I think about it, most of them are. That's a good point that I hadn't thought about, they might just feel weird calling someone younger than them by a title.

The Dr might feel weird being called a title by someone his mom's age, particularly if they also seem uncomfortable about it.
