The publisher then sends this info to the cover designer. A lot of covers are created from stock photos/models, where the cover designer may recolor a gown or the background, or composite some images together. The Big Publishers are more likely to hire an artist to design a cover from scratch, and only authors with a lot of clout are able to specify what they want the cover to look like. At most, a smaller-name author will be able to approve or disapprove the design, but a lot of them won't even see the cover until the public does
Indie authors have a lot more control over this: working with a designer if they hire one, or purchasing stock images themselves.
