Now I’m wondering if any actor has ever requested or been given a significant size boost along with the coverup.

Kristian Nairn, the actor who plays Hodor in Game of Thrones, had to wear a very large prosthetic (14 inches, apparently) because the character is supposed to be half giant. He didn't ask for it but he still got it.
