I'd guess, all of the above and probably some more. Presumably the people who want to read that tag get what they're looking for, at least some of the time.

I've seen Natasha Stark used as an OFC as well: "Tony's sister/cousin/daughter who also happens to be called Natasha and mysteriously resembles Black Widow *and* Tony. And she also bangs *insert author's fave here*" But those tend to get tagged just as Character/OFC with the name only mentioned in the summary.
