I think that it's relatively easy to understand what people who have rape fantasies are fantasizing about - what the content of those fantasies is - and how it differs from an actual act of rape and can be incorporated into consensual erotic play. I think it's a lot harder to understand that in this case. And one of the frequent defenses of underage smut is that "it's all fictional, it doesn't involve real kids". So it's not just a question of sexual acts, but also of fantasizing about real kids (as opposed to fictional kids) being wrong. And I think it's hard to believe that line actually exists in practice as much as some people insist it does.
I want to be clear that I specifically don't agree with OP's implication that everyone who's into these things feels this way.
