I pretty much always hate it because by the time that moment has come around they have likely killed dozens, if not HUNDREDS, of mooks and minions. Video games are the worst offenders, but it happens in pretty much any action media. To me it comes off that the hero only considers the powerful as real people, or only care about people he knows personally, or doesn't value the lives of the working man. It's not a good look unless it's actually (comics accurate) Batman who is super careful not to kill anyone, otherwise it comes out of nowhere and I hate it.
