I hate the "it's REALISM" excuse for why something is the way it is in any kind of media. It's lazy. That game sounds like it was created with the intention of frustrating the player and I sure as shit don't have time for that. A challenge is fine, but intentionally making the mechanics hard and using the "realism" excuse is just dumb.

Realism doesn't work in games, imo. The only time I saw it work was when it was very obviously stretched and turned into "Fake Realism".

the idea NPCs can do things PCs can't, like survive 8 stab wounds, or walk through solid objects, is one of my biggest pet peeves in gaming. It is either lazy programming or an extremely unbalanced game.

Ok, before you come at me, it is ONE thing if it's a big boss and you've got this party you've been leveling up. Every "day" NPCs or animals/monsters shouldn't be so far above your level you have no chance. (And the walking/running through solid objects is just a big peeve. Look, if I'm chasing you, you have to go around the tree, just like I do, or you can't go up that hill because it's too steep, just like me. Otherwise, the game is unbalanced/lazily programmed.)

I'm kinda confused what you mean by "balance" here. To me, it seems like the only possible useful understanding of "balance" in a game context is whether the distribution of challenges in the game is rewarding and satisfying for the person playing the game.

Now, if the game is too challenging and not fun, I agree that's bad balance. But comparing PC and NPC stats seems like a totally arbitrary way of measuring how challenging or satisfying or rewarding the game is. They're totally unrelated!

Realism is always fake. Realism is a literary effect and a rhetorical technique.

That doesn't make it bad but also, it sounds like it's probably not something you're into in video games (and TBH neither am I)

What I hate the most is when survival games are supposed to be realistic but you just need to stuff your face with food every few seconds. I am more okay with it if it balances that with healing wounds more quickly than would ever be possible irl but if they do the whole "one wrong scratch and you die because you bleed out or it gets infected" while making me feed the character far more often than would be necessary I'm out.
