At first glance, I did not realise that the big guy was holding and eating another guy. I was wondering why the big guy's penis was sliced in two and had the balls apparently dangling from the tips. And then a part of my brain just went what a strange fetish and moved on, as if that were that.

Uh, I think it can still be a unique setting and good world-building even if it is just based on some real world culture. Depending on the author. Complete and total originality is not the be all and end all of good fantasy.

Maybe it's just me, but it doesn't seem to me like OP thinks it's not good because it's based on a real world culture, OP is saying that the details taken from the real world culture are largely superficial, like borrowing place names and that's the not so good part.

Hmmm. It sounded to me like the OP was saying it wasn't good because the authors weren't changing the cultures to make them more original, they were just basing them on real world cultures wholesale without any alterations.
