I just can't stand how we're supposed to sympathize and agree with the idea that the autistic wife was a Problem and how romantic it was when the husband clearly kinda treated her like shit and oh, gee, isn't it Romantic that she still shows up in the end?

I also think the whole situation felt forced, but my take is that I really don't like the whole thing about him not remembering due to the trauma and medically induced amnesia his mom forced on him with the pills to make him more controllable and him just having too many issues to make life passable for himself much less be together with another person who also has a lot of special needs. I don't think the game meant for River to be "the problem". Her condition made it impossible for her to outright speak her mind so she tried her best to make Johnny understand, who himself was unable to understand because he just could not ever remember. They are both the problem, that's the whole drama. It was impossible for River to make him remember, which she didn't know, because Johnny could never remember and that made it impossible for him to ever understand her hints. That she appeared in his death dream world/life was not guaranteed but made sense, not only because it is oh so romantic but because she was the reason why he wanted to go to the moon in the first place. His wish to go to the moon is supposed to show his love for River, a promise made in a time he can't remember but that is so important to him that his subconsciousness still retained her in that way.
I still think it's stupid that River preferred to deny medical treatment in favour of financing that house. No matter how important it is for her that Johnny remembered, her life should be more important. And Johnny was just too much of a trauma pile-up and with his identity crisis had too much going on to focus on River on top of everything. That's why it's my least favourite as well, it's too much outright frustration and unsolved issues. River dies unhappy and unable to make Johnny remember and Johnny gets his happy death dream but it feels wrong and the only "right" things are River and his dead brother, he finally remembers. But at this point it's arguably too late, unless you count it as a win for Johnny in his last moments at least. Which I don't.
Ultimate he might have died happy but it doesn't change the fact that it was a lie and his real life was miserable as well as the real River's.

But all this is also something that the Game series tries to show. Giving people their dream lives has many issues. Is it right or wrong from a moral and ethical standpoint? See the minisodes and Finding Paradise that pick up on that perspective.
