It depends on author voice--in some narrations, using the word "boob" would fit perfectly but using other words would be OOC.

I can understand having an aversion to the word (for me, the word "buttocks" always throws me out of the rhythm of a scene) but your preference does not equal a concrete rule for everyone else. So yeah, don't say anything.

Character voice is the deciding factor, IMO. Dean Winchester very well might think "boob." Aragorn probably would not.

Yes, this. There are some characters who would 100% think of it as a "boob" and so if the narration is in their inner voice, it would make sense.

I wasn't able to find a Sindarin equivalent but in Quenya it appears to be ambos or ambar (depending on derivation)

+1, for some characters it would be totally appropriate and the secret poster's reaction is there problem. Other times...no, the writer is in the wrong.
