For some reason I am reminded of how the Uruguayan rugby players that survived were not initially taken seriously as having endured starvation and resorting to cannibalism because they didn't look horrifically emaciated after the fact. I wonder if this guided the creative team's decision for this show.

This was my first thought too. Also, look at how many people have anorexia for months or even years before the people around them realize it because they don't look obviously visibly emaciated.

it can take a long time for starvation to appear on somebody's body -- even if they lose weight, they might not lose their 'shape' for a long time. it usually appears on their face more quickly, which is why contouring/makeup is a good solution here.
