Most places only accrue sick days/personal days, so you don't start with any. It sucks.

I work in healthcare, not in the US, and we absolutely have sick days...but there is massive, massive pressure not to take them because we (and pretty much all medical facilities) are so horribly understaffed that taking one piece out of the toppling stack is actually a serious problem. It's horrible, and you get guilted into it. I haven't had COVID since the restrictions lifted, but I've had two co-workers pressured into coming in while still testing positive, and they did.
