I knew that fine could also mean sexy/hot, but I was not aware of the nuance when using it as a question.

Honestly, with English as my first language, I wasn't aware of the nuance either until I started seeing a lot of "Are you fine?" in my first Chinese fandom. And it bugged me because that wasn't something I had ever heard before and I kept mulling over it until I could figure out why we don't say that in North America.
