The same thing I hate IRL... characters who repeatedly make bad choices, refuse to take responsibility for them, and try to blame everyone and everything else for what happens.

The difference for me is if canon acknowledges it. Villains who make all the bad choices? Sure. Heroes who make lots of bad choices but learn in the end? Fine. But heroes who make all the bad choices, blame everyone else, and canon thinks they are right to blame everyone else? No thank you.
