There's a browser extension for Chrome (and any Chromium-based browsers) called XKCD Substitutions which could do this for you. It's based on XKCD 1288 (https://xkcd.com/1288/) and XKCD 1625 (https://xkcd.com/1625/) but also includes the ability to add your own custom substitutions. The defaults are deeply amusing, in my opinion, but can certainly get confusing if you forget the extension is on!

Not an app, but if you're reading fic in a browser that can support extensions it might be sufficient for you.

OP - Thank you! I thought I'd seen someone on my tumblr feed mention something like this several years ago, but I had absolutely no idea how to look into it. And I actually prefer my laptop over my phone for most things (as befits the fandom old I am), so this is perfect!
