What if there were countries instead of cities and there were distinct cultures on the Sword Coast and also the gods fucked hard?

It gets the fantasy/sci-fi balance completely wrong - too much SF given the fantasy focus.
Starfinder gets it right...and also makes Golarion a lost world, which I don't think is coincidental.
