I took it as a vent/hyperbolic piece of art rather than trying to make a statement on mental health, though I don't know the artist's intentions. The comic seems to show that others do find the cat tiring and the cat has trouble fitting in, with neither exactly portrayed as wrong. The cat is portrayed as difficult to get along with and isolated due to their sensitivity. I thought the part about not knowing if others have hearts because they're hidden was less judgemental and more about struggling to understand others' intentions if they are different from you, sometimes assuming the worst. I can see why it rubbed OP and others the wrong way, the judgy interpretation is another way to read it and I can see reading it with a condescending tone. For me though I just took it as a comic about the experience of being emotionally volatile.
