For me the majority of fandoms I'm into are usually good at tagging fics in a way where if I did want to avoid transmen or omegaverse stuff, I could.

The only time I had come across an untagged AU fic if characters being trans was when the writer had a rant in their notes about how people should just deal with the existence of trans folks, and overall it came across as a very young person wanting to vent about the transphobia in their lives.

Of course there's also people not thinking to tag everything accurately, but that small tidbit is my 2 cents to this.
