I don't play anything, but my understanding is that sometimes (often?) the mismatch between the motions of the actor and the music they're meant to be playing is a sound design/directing issue, where the actor learned the correct motions for one specific measure for a close-up and then someone decides they'd rather use a different part of the same piece, or a different one altogether, but keeps the same footage.

Of course it's still annoying when no-one even tries, though.
