A lot of times, it seems like it's a character who is unconventionally attractive, and doesn't fit mainstream society's idea of beauty but has a pretty rabidly horny fanbase of those who do find that person attractive. When I see "I'd hit it"s about those kinds of people, I just assume the poster is new to the fandom and has yet to stumble upon that fanbase.
