It can work that way in a lot of other countries, too - medicine as a graduate degree is getting more and more common even in places that traditionally do it as an undergrad course. It brings in a wider range of people with more life experience who are likely to make better doctors than simply selecting from a bunch of 18-year-olds with high marks. You still have to pass the test to get in (meaning you either have to have a degree in or have personally studied the appropriate science and maths) so you're not starting from zero and it's very common for people to do an associated bachelor degree then go onto medicine.
