Sometimes, yes. Occasionally I bind up a whole book from scratch, like this Japanese stab-bound take on Roger Zelazny's "24 Views of Mt. Fuji by Hokusai" (hope the link works!): https://www.facebook.com/media/set/?set=a.855161904529813&type=3

These days it's often more a matter of patching up existing books, which is comparatively dull (mending tissue on torn pages, gluing split corners-- there are a couple books here I may have to completely take apart and rebind because the signatures are coming apart).
