From what I've seen of hentai, variants of femdom are relatively common, although nowhere near as prevalent as male dom/female sub. "Milking" is also an acknowledged fetish, as is actual pregnancy kink.
However, in the vast majority of cases (not all, just most), the whole "oh nooo you'll get me pregnant" trope comes up when the girl/woman is in the submissive position (or even a victim), and the boy/man dominates her (with his mighty penis and/or inexplicable sexual prowess). Thus, it is his decision whether she'll become pregnant (let's disregard biology, this is hentai) and her body is utterly subjected to his will, not hers anymore.
This is why it always seemed to me that in most cases, it wasn't really an impregnation kink, which - as you said - is something entirely different.
