I think it is a pretty darn horrifying concept. A traffic jam of millions of people. A traffic jam of millions of people who have been living for years and years in a space the size of a VW camper van.

You can't go outside for a walk, there is no point even in sitting near a window because there is no view. No escape, you can't go back. You can't meet other people, you can't have a hobby, you can't have personal space. You can't even stand upright. And if He hadn't came along that would have been your life forever. In fact given it has been like that for years there must be "cars" that just have dead people in them. Died of starvation(?), or of neglect, or old age, or accident. Worse imagine you are in one of those cars and your SO has a heart attack, you try calling for help but get placed on hold and they die. And you have to spend the rest of your life with their corpse or chuck it over board.

How many kids were born knowing nothing else, how many mothers died in childbirth? Imagine a pregnant single woman at the start of the jam, she has to give birth on her own, and even if she survives and takes care of the kid for a few months; one day she slips and strikes her head. She dies, and the baby dies of starvation, crying out in a tin box for help that will never come.

And one more thing. None of those people in the jam expected it to take so long. At the start it was just the normal commute to work/friends/whatever, then daily life stopped and all there was at the jam.

based on some of the cars the Doctor drops through on his way to the express lane, some of the people have gone stone mad from the isolation. At the end of the ep when those cars fly off into that brave new world, how many can actually cope with their new "normal" lives? If we ever returned to New Earth, how many people would have driven back to the motorway simply because they could no longer cope with anything else? According to the show traffic has been sitting for twenty years.

The episode didn't make me jump, as the OP put it, but when I thought about the bigger picture is really is terrifying.
