My mate is a sex worker & while a lot of the time she's just thinking "oh, I forgot to vacuum yesterday", that's not always. She does get a good number of clients who she does have a good time with. She's told me a lot of the guys over 35 are great & go out of their way to make sure she enjoys the sex too. And she has a few regulars that she genuinely likes & enjoys sleeping with them.
So yeah, it's not always "when's he going to finish?" =)
