The poor man could have a knife sticking out of his chest and the fic focuses on how Sherlock is the one really suffering because he has to see the knife sticking out of Johns chest, won't sleep or eat until the knife is out of John's chest and will have PTSD because of the scar left by the knife in John's chest.
