Basically, Robin has the ability to create copies of/multiply her body parts, see the picture posted by the commentor above. You can google Nico Robin + One Piece for more info on how it works.

If I understood the snippet in the secret correctly, Robin created a hand on the inside of someone's thigh, probably to give him a handjob.

Actually I'm pretty sure that fic was...*coughs* about her growing boobs on Luffy (it was for a disguise or whatever) and him exploring (with his mouth), which sent the sensations back to her. I've read weirder things, sadly, but OP's right in thinking its a rare kink. You probably wouldn't get it outside her character.
