Depends on the story. It's usually written as a genetic quirk, sometimes one that's always been there, or sometimes one that arose because of... reasons. They don't always go into details on the reasons, but they are related to female fertility a lot. I've also seen fic where it's only found in dog/human hybrids. Oh, and it shows up a lot in werewolf au.

I've actually seen some omega!fic that has interesting explorations of gender issues, but a lot of it does tend to be used as a way to write mpreg/heat/self-lubricating anuses without bothering too much with backstory and logistics.
