I'm... baffled that OP chose Shinji/Kaworu as an example of healthy and loving relationship.

I think that last sentence makes it obvious that they know that their relationship is fucked up. Though their relationship isn't very healthy, Kaworu is the only "person" Shinji has met who genuinely cares about him and doesn't want something in return, and I think that's the part OP was focusing on, not...everything else about their relationship.

I don't know. I've only read the manga, but to me it didn't seem like Kaworu was in love with Shinji - more like a combination of fascination, curiosity and jealousy. As for wanting something in return - he did ask Shinji to kill him, didn't he? Even though he knew Shinji wouldn't be happy about that.

And can't help you about the icon, sorry - I never actually tried to use any but my default :D

I'm only familiar with the anime, so maybe this is part of it. From the little I've seen of the manga, Kaworu is very different in it. He's only in one episode of the anime, but he seems to have a generally pleasant disposition and actually does like Shinji. Also, he wants to die so Shinji and his fellow humans can live, so it's still somewhat caring in a very fucked up way. In no way is their relationship healthy, since Kaworu's ways of showing he cares are really warped, but he seems to really want to be Shinji's friend.

That was more meant as a general comment, about the icon, not specifically to you, so I'm sorry that was unclear. But, my icons work elsewhere...IDEK about LJ anymore xD

The anime characters, which came first, bear little resemblance to the manga characters (Kaworu doesn't strangle cats for one). Kaworu is only in the one episode but he's very loving and caring toward Shinji (it's been years but iirc, he told Shinji that he was created for Shinji and he pursued a relationship w/Shinji--they had a sleep over even), who doesn't really know how to take it but yearns for it and is starting to accept it when he has to kill Kaworu, who ends up sacrificing himself and his mission in order for Shinji to live--he wants Shinji to keep on living and that's why K must die by S's hands. Having to kill Kaworu really fucks up Shinji and impacts the rest of the series. No one has sympathy for Shinji having to kill what he regards as the one person who truly cared for him w/no expectations (ie. I don't want you if you won't kill the Angels attitude of Misato and Gendo (Shinji's father)).

I'm always baffled but grateful whenever someone explains those last few episodes of Evangelion. I never understood what happened.

It's not so much that their relationship is in any way healthy as it is that it's less unhealthy than practically every other relationship in the series. But that says more about the other relationships than about theirs.
