I think the K-mart thing is only an Australian and New Zealand thing. I did it a couple of years back, and you basically just take a tag from the tree that will say "Name...", "Age..." and "Gender..." and you pick out whatever gift you want to buy, fill out the tag, then put it under the tree with the gift.

Then the day before Christmas, all the presents are collected by the Salvation Army. They are taken to a big warehouse, and evenly distributed amongst the towns that need them the most. Then the underprivileged parents come into their local Salvation Army store, and pick out free gifts that they think their kids/family/etc will like. So instead of a stranger picking out a random gift for an underprivileged kid, it's the parents who get to choose what gifts they want to buy for their child. :)
