I think you would have to work awfully hard to categorize the modern US as a "tyrannical hellhole." Like, really, really hard. And that's long before we get into the absolutely ridiculousness of comparing a health care system you don't approve of, or the execution of violent convicted criminals who had lawyers and trials, to putting innocent children into an arena and forcing them to kill one another off for entertainment. Regardless, in either case, of the rightness or wrongness of the practice itself, neither one is remotely comparable to the Hunger Games or Panem. Honestly, there is so much wrong with your whole first paragraph that I'm struggling to work out how on earth you got to thinking it would be logical in the first place. It is honestly, genuinely, stupidly offensive.

You're looking at this and going "Well, no country can get through the Capitol's military, and there'd be diplomatic/military repercussions if they tried." Which is probably true when looking at military solutions. And certainly militaries tend to be the fastest and most effective at getting relief supplies to a particular place where they are needed. But government-backed relief, or officially acknowledged help, is not the only kind out there. Nor even, always, the best. People send help to Darfur all the time. Doctors Without Borders goes to all kinds of places, even where the local government may not necessarily approve of their presence. Missionaries don't always stop at border crossings, either, when bringing--aside from their message--medicines, food, clothes, or other relief supplies. Coca-Cola (yes, the company) sometimes packs relief supplies to various places in with their regular shipping.

Simply put, when people see genuine suffering, somebody is always motivated to help, if they have the ability to. And the danger/difficulty of getting there to do it doesn't stop everyone. (Which is also where your "The USA is a third-world dictatorship" allegation really goes to bits; if Europe really thought it were that bad, they'd be sneaking help in to us.) And private organizations like this are deniable by the governments of the citizens involved, meaning the risk of diplomatic/military repercussions is much smaller or non-existent if the people in them get caught. If some Canadian missionary goes to a country where the local government gets mad at them, the Canadian government is not responsible for their actions, even if they tacitly approved of them. What most of the organizations and people I mentioned have in common, though, is that their efforts are low impact. They don't reach a lot of people. So in the Hunger Games, most people would never be affected by them. And by the time word got around to anyone (especially from district to district) it would be just a rumor, if it got there at all. But Katniss would've heard that rumor eventually, going to the Capitol, and 13, and visiting all the districts. And, instead, there's nothing. No rumors, no stories, not even guesses. The possibility just doesn't exist. And Panem could really use some help from Doctors Without Borders, among other folks. But apparently nothing ever came. So, my conclusion is still that either no one is out there to send help, or whoever is out there is fairly badly off themselves and can't.
