In a vacuum, I totally agree with you. But in the social climate in which most of these comments are made, it's considered extremely insulting to say a woman looks masculine. It's basically a specific kind of ugly when applied to women. So unless somebody specifically spells out (as you did in your comment) that they are deviating from the social norm, which is that they mean "manly" as an insult when applied to a woman, I will reasonably assume that they are being insulting.
It wouldn't be so bad if women were not considered worthless when they're ugly (again by society at large/TPTB, not necessarily by any individual).
