I don't really think it's insulting to say someone looks like a man. Or mannish. Or masculine. It's just an observation. Of course, if the person people are talking about comes out and says that they feel insulted by it then that's different. But why does everyone assume when people make a comment about her masculine features that it's an insult? I think she's very masculine and pretty.

Um because it's normally used to degrade and put down women who don't fit the traditional idea of femininity?

In a vacuum, I totally agree with you. But in the social climate in which most of these comments are made, it's considered extremely insulting to say a woman looks masculine. It's basically a specific kind of ugly when applied to women. So unless somebody specifically spells out (as you did in your comment) that they are deviating from the social norm, which is that they mean "manly" as an insult when applied to a woman, I will reasonably assume that they are being insulting.
It wouldn't be so bad if women were not considered worthless when they're ugly (again by society at large/TPTB, not necessarily by any individual).

Even under the circumstances you spelled out, I would be very insulted if you said that to me. I am a woman. I am not a man, I never have been, and I never want to be. There's nothing wrong with men, but I'm not one.

Because that's kinda like saying if someone's face is shaped a certain way, they're less their gender? If you are a lady and have very strong features, that doesn't make you less woman/ more man than another lady who has more delicate features. Some men and women like androgyny, but most want to be perceived as what they are.
