Well, if you're worried that Adam's thoughts might be too obtrusive in the scene, maybe it would work better to show him checking in with her once or twice. It could be a verbal safeword system, where she says something like "green" to signal that everything's all right or (and I'm a fan of this) it could be something like having her squeeze his hand as an "okay" signal.

I don't know how long your fic is, but you could also show some of the negotiation between them before the scene. In my experience, a lot of rape roleplay fics will have some tenderness and frank discussion between the characters before and after the rape scene, which can give the writer more latitude to stay "in-fantasy" for the scene itself.
