The Doctor said that it was important to hold on to feelings of love to stave off the infection--and since Rory feels as if he has MORE love than her (and, as other anon pointed out, he has reason to), it would take slower on him. It would SAVE HER LIFE. He was trying to make her understand that she didn't need to take his wrist thing out of some token of love (since he believes she no longer loves him), or affection, but out of simple, as he put it, arithmetic. He loves her more, here is why.
So this is an exceptional circumstance that he brings it up.
